Our leasing office remains operational and we are pleased to offer virtual leasing options for your convenience. Applying for housing online is quick and hassle-free. Simply fill out the application below as completely as possible. The information you provide helps your leasing consultant find a home that best meets your family's needs. Please note in order to be placed on the wait list, a copy of the service member's orders must accompany the online application. In addition please review the list below for documents you can submit to get your application processed faster.
Once you have submitted your application a leasing consultant will contact you within one to two business days. (Please note if an application is submitted on the weekend it may take slightly longer.)

For Active Duty Service Members:
Please upload a copy of your orders** with your Soaring Heights application.
In addition, please complete the following documents for HMO:
Active Duty Referral Packet - Privatized Housing Referral Form, Application, AF Form 1442 and Housing Briefing Sheet*
*A referral from the Housing Management Office (HMO) is required to complete the processing of your application. Please download the referral packet and complete the highlighted fields. The completed forms can be uploaded with your online Soaring Heights application or you may scan and email the forms along with a copy of your PSC orders to 355CES.HMO@us.af.mil.
** Additional Information: If your dependent(s) is/are not included on your PCS orders, additional supporting documentation may be required such as: DEERS Verification of Dependents (DD Form 1172-2) from MPF, marriage certificate, pregnancy verification with expected delivery date, 60-day prior to marriage letter signed by Commander or First Shirt, custody paperwork, etc.

Background Check Release Form (click here to download) - (There is a $30 fee per applicant for any person over the age of 18; a leasing consultant will contact you to collect payment).

OET Referral Packet - Privatized Housing Referral Form, Application, AF Form 422 and Housing Briefing Sheet*
* Obtaining a referral from the Housing Management Office (HMO) is required to complete the processing of your application Please download these documents and complete the highlighted fields. The completed forms can be emailed to 355CES.HMO@us.af.mil or dropped off in person to the HMO Office located in the Welcome Home Center at 8090 E. Ironwood Street, Davis-Monthan AFB, AZ 85708.
